Michael Moore.com : Take Action : VOTE!!: "10 POINT ELECTION DAY CHECKLIST
(see also: WHAT YOU NEED ON ELECTION DAY: A State by State Guide From BringYourId.org)

1. BRING PHOTO IDENTIFICATION
You absolutely must bring a photo ID to the voting booth. It should be an ID issued by your state or the federal government, such as a drivers license or a passport. This is a must. DO NOT FORGET YOUR PHOTO ID - bring two if you have to!

2. BRING PROOF OF ADDRESS!
You know that bill you haven't paid that's sitting on your desk? Grab it and bring it with you. A credit card bill, a utility bill, a cell phone bill?whatever it is, nothing proves you live somewhere quite like a statement of how much money you owe someone else for something. Bringing a bill with you will demonstrate that you do indeed live where your voter registration information says that you live.
If you have recently moved then you should bring proof of your new address as well as proof of your old address. Also, some states will not let you vote on all races if you have recently moved into their state. You will still be able to vote in federal elections, for President and Congress, but you may be given a "limited ballot." If you have any questions about this call 866.OUR.VOTE (866.687.8683).

If you are in college and do not have a formal piece of mail confirming your address you should be able to go to your school's administration offices and get a brief letter confirming that you do indeed attend that school and live at your address - make sure it is on official school letterhead. "
